1COOPHealth and Maxicare as the New HMO Providers
The CBMPC Board recently approved to engage the services of two Health Maintenance Organization (HMO) Providers for its members: the Cooperative Health Management Federation (1COOPHealth) and the Maxicare Prima by MaxiHealth covering the period October 16, 2025 to October 15, 2026.

The 1COOPHealth is the first duly licensed cooperative HMO in the Philippines. Formerly regulated by DOH from 2015 to 2017, it was transferred under the regulation of Insurance Commission from 2018 to present, while being regulated and governed also by the Cooperative Development Authority.
1COOPHealth is accredited with the top six major hospitals such as the Asian Hospital, Cardinal Santos Hospital, Makati Medical Center, The Medical City, St. Luke’s Medical Center in Quezon City and in Global City, and with all minor hospitals or a total of 1,914 accredited providers nationwide as of August 27, 2025.
The 1COOPHealth premiums are according to the age of enrollees and room type with corresponding Maximum Benefit Limit per illness per year, as shown below:


Any availments in excess of approved maximum benefit limit should be settled directly by the enrollee to the doctor/hospital/clinic concerned.

Maxicare Health Services Inc. (MSHI) is a subsidiary of Maxicare Healthcare Corporation (Maxicare) which provides quality and accessible medical services to its clients of all ages. To date, MaxiHealth has 34 primary care clinics strategically located nationwide.
It is committed to helping Maxicare’s clients live their best lives through a wide range of healthcare services, including preventive care, primary care, and specialty care. The company offers state- Clinics (PCCs). Transactions are paperless with no Letter of Authorization required. Members just have to present their prepaid cards to any PCCs when availing of their services.

Exclusions and Limitations:
Outpatient consultations and procedures outside PCCs
Hospital admission or confinement
Emergency cases
Eligible enrollees to both HMO Providers are the CBMPC members. Non-CBMPC members can also enroll provided they apply for membership at CBMPC and subscribe/pay the initial share capital of at least P5,000.00 and membership fee of P500.00. Enrollees who are not qualified to become CBMPC members shall pay an affiliation fee of P1,500.00.
Payments on the corresponding premium can be made either via on-line deposit/fund transfer to CBMPC’s account in LANDBANK/BDO or over- the-counter at CBMPC office at Unit 205 One Taft Residences Cityland, 1939 Taft Avenue, Malate, Manila.
Effectivity of HMO coverage will be upon receipt of acknowledgment/confirmation from both HMOs of CBMPC’s enrollment. Once enrolled, both HMOs will not allow change of plan type (upgrading/ downgrading) and cancellation. NO REFUND OF PREMIUM shall be allowed resulting from withdrawal of enrollees.
