We are glad to inform you that the Countryside Builders MPC’s Board of Directors approved on August 24, 2023, via referendum, Value Care Health Systems Inc. as our HMO provider for the period September 1, 2023 to August 31, 2024.
Hereunder are the features, guidelines and procedures:

III. GUIDELINES:
A. For those new enrollees (Principal and Dependents) Maximum Benefit Limit (MBL) will be up to 150,000 ONLY.
B. For DEPENDENT members: Pre-Existing Conditions covered up to 100% per illness, per year.
C. The benefits under this Policy are supplement to the benefits under Philippine Health Insurance Corporation (PHILHEALTH). This Policy shall reimburse covered expenses up to the maximum limits, which are not payable under the said PHILHEALTH. In no instance, however, shall the total payments under both plans exceed the actual expenses incurred.
D. Enrollee/s without PhilHealth Coverage may opt to:
- Enroll voluntarily under PhilHealth
IV. PROCEDURES
- Enrollees should signify their intention to enroll in a plan type of their choice by filling up the attached Enrollment Form or you can access the form thru our Website: cbmpc.org

2. Enrollees can pay the corresponding premium by:
2.1. Online deposit or Fund Transfer to:
- Countryside Builders MPC’s A/C no. 34021011-63 at LandBank Plaza or thru any: LandBank branch nearest your area.
- Countryside Builders Multi Purpose Cooperative Savings Acct. No. 004370045056 at BDO at Pedro Gil A. Mabini Manila Branch.
- For Online Banking Transactions please send us screenshot of the confirmed/successful fund transfer.
2.1.1 Deposit slip should clearly show printed name of the enrollee/s name of LandBank/BDO branch, and amount of premium paid duly validated by the teller.
2.1.2 The said deposit slip should be forwarded to CBMPC either by email address: [email protected], & [email protected], FB MESSENGER: Cbmpc Coop II & Marites “Thex” G. Manansala.
2.2. Over-the-counter at CBMPC office at Room 206 Pius Catholic Center U.N. Ave corner Correa St. Brgy. 664-A, Paco Manila
V. Instructions/Conditions
- Deadline of payment on or before August 30, 2023 ( NO PAYMENT, NO ENROLLMENT)
- Effectivity of HMO coverage will be upon receipt of acknowledgment/confirmation from Value Care Health Systems Inc. of CBMPC’s enrollment.
- Once enrolled, Value Care Health Systems Inc. will not allow Change of Plan Type (upgrading/downgrading) and Cancellation. NO REFUND OF PREMIUM resulting from withdrawal of enrollees.
- To avail of the HMO coverage, applicant should be a CBMPC member, Non-member must subscribe and pay the required minimum share capital of P5,000 and membership fee of P500.
- ANY AVAILMENT, MADE IN EXCESS OF ALLOCATED MAXIMUM BENEFIT LIMIT (MBL) SHOULD BE SETTLED DIRECTLY BY THE ENROLLEE TO THE DOCTOR/HOSPITAL/CLINIC CONCERNED.
